She was born into a Mudaliar family in Tiruvarur district of Tamil Nadu.
Her family later relocated to Mannargudi.
She married M. Natarajan, who was a temporary Public Relation Officer in the Tamil Nadu Government.
In 1976, her husband lost his job during the Internal Emergency enforced by Prime Minister Indira Gandhi, leading them to sell or pawn their jewels for livelihood.
To supplement their income, Sasikala started a video renting business.
Her husband had a connection with V S Chandralekha, the District Collector of Cuddalore, who was linked to M. G. Ramachandran, the Chief Minister of Tamil Nadu then.
V S Chandralekha introduced Sasikala Natarajan to Jayalalithaa, and they developed a close relationship.
In 1995, Jayalalithaa organized an extravagant marriage for her foster son V. N. Sudhakaran, Sasikala Natarajan's nephew.
Jayalalithaa faced criticism for the lavish spending at the wedding.
After Sasikala entered Jayalalithaa's Poes Garden Residence, she was accused of misappropriating funds on Jayalalithaa's behalf.
On 19 December 2011, Jayalalithaa expelled Sasikala from AIADMK, but she was reinstated on 31 March 2012.
Following Jayalalithaa's death on 5 December 2016, Sasikala was poised to succeed her as AIADMK General Secretary.
She became the 2nd woman to lead AIADMK after Jayalalithaa when appointed as General Secretary on 31 December 2016.
